CASE MANAGEMENT

Each individual is assigned a Case manager who is responsible for:

¶     Coordinating the development and implementation of each person’s Individual Service Plan

¶     Coordinating the delivery of each person’s Individual Plan of care

¶     Coordinating and monitoring the delivery of waiver and generic services

¶     Integrating various aspects of services delivered under the waiver and through other sources

¶     Recording each person’s progress

¶     Pre-discharge plan development

¶     Record keeping

¶      Arranging transportation

RESIDENTIAL ASSISTANCE

Each individual is given the opportunity of choosing their type of residential assistance from the following options:

¶      Supported Home Living – this service type is provided to individuals residing in their own residence or the residence of their natural or adoptive family or to individuals receiving foster care services from the Texas Department of Protective and Regulatory Services or the Texas Department of Human Services.

¶      HCS Foster/Companion Care – this service type is provided to individuals in a residence by a foster companion care provider exclusive of Texas Department of Protective and Regulatory Services or Texas Department of Human Services foster care programs.

¶      Residential Support – This service type is provided to individuals by daily shift staff or by individual staff member with periodic relief staff.
